The cover first popped up at the intersection of Bay Street and Whalley Ave. in July, covered with a thick patch of dirt.
The city has said the cover on the Whalley Avenue bridge is not related to the pavement issues discovered on the Ballantyne overpass.
When it was first discovered, city officials estimated the cover at $2,000. City Staff told CTV News back in July it was trying to determine what happened and the cost of the repairs.
A city release on Wednesday from Wednesday said as with the other cover, that cover was reinstalled in a different location.
This time it was found in place of the right side of the intersection just before Bay Street intersects Whalley Ave. at Kensington Oval.
The city said it is unclear what caused the cover to fall, or what the cost of the repairs will be.
“What’s interesting is that we had a subsequent collapse almost exactly one year ago on the Whalley Avenue bridge,” Mayor David Hogg told CTV News on Wednesday.
“So, that’s suspicious and we’re going to be looking into that, just like we did the last one.”
The police investigation also is being conducted in the area around Kensington Oval, where the two manhole cover problems occurred.
A review of traffic camera images showed a yellow patrol car traveling along Whalley Ave. shortly before the mishap, and shortly after a blue “Saskatchewan RCMP” vehicle followed.
“Our officers were standing in the area about five minutes or so after (the collapse),” says Det. Const. Bradley Dobson, who is heading the investigation into the incident.
There was no immediate information whether the cover was damaged or evidence left behind leading to the investigation.
Hogg said the city cannot share the cost of the repairs, despite the two collapses.
He said the city is trying to determine who owns the cover and the other cover
